三、评价主体和方法 （一）评价主体
对学生综合素质评价采取学生自评、家长评价、同学互评、教师评价相结合的办法。评价主体为学生、班主任及学科教师。

1、学生自评：学生通过自我陈述、自我展示、自我鉴定等形式对自己三年来的成长过程、所取得的成绩及今后努力方向进行充分展示和自我评价，并给出自评等第。
2、家长评价：学生法定监护人对学生三年来的家庭和社会表现实事求是地反馈，并写出评语。

2、在道德品质与公民素养评价中，学生若非涉嫌犯罪受到过司法机关处理或因严重违纪受到过校级记过以上处分而未撤消者，该项评价结果应为D 等以上。
六、评定工作的制度建设 （一）公示制度
